Gathering feedback from participants in the game is really important to us. This is how we will know whether the practical steps are making an impact or not. And that's why we'd like you to fill out the online survey.
Please click the appropriate survey link below to have your say on current behaviour in football. We are interested in hearing as many views of coaches and managers, captains and players, spectators and referees as possible.

All survey participants will be entered in to a draw to win one of five pairs of tickets to England v Slovakia at Wembley Stadium in March 2009. Please note the survey is only open until 31 October 2008.
